Medical and Telehealth Consent

This consent explains telehealth care, patient responsibilities, potential benefits and risks, medical decision authority, privacy expectations, and emergency limitations.

Emergency warning

The Perstet website and telehealth services are not emergency services. If you may be experiencing a medical emergency, severe allergic reaction, suicidal thoughts, chest pain, difficulty breathing, stroke symptoms, or another urgent condition, call 911 or go to the nearest emergency room immediately.

What telehealth means

Telehealth uses technology to support care when the patient and provider are not in the same physical location. It may include online intake forms, secure messaging, phone calls, video visits, photo or document uploads, review of medical records, electronic prescribing, laboratory coordination, follow-up education, remote monitoring information, and other electronic communications.

Potential benefits and risks

Telehealth may improve access to care, reduce travel, make follow-up easier, and support convenient communication. Risks include technology failures, delays, incomplete information, inability to perform an in-person exam, poor image or data quality, limited medical records, possible drug interactions or allergies, privacy incidents, prescribing limits, and the provider deciding that in-person care is required.

Provider discretion

A licensed provider decides whether telehealth is clinically appropriate and whether any diagnosis, treatment plan, prescription, laboratory order, or follow-up is medically appropriate. A provider may decline treatment, require more information, request a video visit, refer you to in-person care, or stop telehealth care when clinically necessary.

Your responsibilities

You agree to provide complete and accurate information about identity, age, location, health history, medications, allergies, symptoms, labs, pregnancy or breastfeeding status, other providers, and emergencies. You agree not to omit information or request medication for another person. Clinical decisions may rely on what you provide.

Privacy, records, and consent withdrawal

Telehealth involves electronic communication of personal and health information. Reasonable safeguards are used, but no communication method can be guaranteed risk free. You may request copies of records as permitted by law, and you may withdraw telehealth consent by written notice, though withdrawal will not affect actions already taken in reliance on consent.

Medications, labs, pharmacies, and financial consent

Perstet is not a pharmacy and does not guarantee that a provider will prescribe medication. Certain treatments may require labs, identity verification, follow-up, or monitoring. Compounded medications, when used, are not FDA-approved and are prescribed only when a provider determines they are appropriate. By continuing with a paid service, you authorize applicable charges according to checkout and policy terms.
